The admin interface allows you to construct a table of ranges of subtotal values with corresponding insurance charge. You also can select to make the insurance cost a taxable entity.

Limitation: This table is based on the subtotal and is not related to the method of shipping. If your shipping methods do not charge insurance uniformally, this module will not be able to differentiate between them.
